2/11/2023 EnchantaVaughn Approved Behavior: During the session, the QMHP attempted to help the client regain and regulate his emotions and feelings, as he attempted to shut everyone out. QMHP continued to build rapport with client and his mother. Mother continued to express to QMHP on how she really wants to get his anger under control so that she will not have to continue to get in trouble at school or in the community. Behavior: The client was unable to focus and complete the assigned tasks that were being provided to him during the session. Client exhibits fidgety and inappropriate body language when communicating with QMHP. QMHP continued to work with client on ways to improve his behaviors and control his anger through the use of counting to 10 or speaking with someone who in authority. Intervention: QMHP inquired about client’s recent experiences in the home. QMHP attentively listened to client’s issues and he discussed his problems. QMHP reeducated client on the dangers of involving himself with activities that are both negative and harmful to his mental health and physical health. QMHP worked with client on methods and techniques to help him reduce, lesson, or eliminate his impulsive conduct and as well as to be more aware of his unpleasant behaviors within his daily functioning life. Intervention: QMHP gave client the opportunity to express and explore his perceptions of how his symptoms of ADHD can affect his relationships with his peers and adult figures. QMHP retrained client on anger management techniques, as a way to help him model appropriate ways to manage his anger. QMHP reviewed methods and tactics to help client positively advocate for himself by the use of assertive communication and conflict resolution skills daily and as well as to assist in reducing physical aggressive behaviors toward himself and others. Response: Client exhibited an unemotional and blank look on his face, indicating he was not in the mood to process or reflect on his day. Client was reluctant and hesitate, as he remained mistrustful in regard to extending a friendly or respectful rapport back to the QMHP. Client appeared disinterested in regard to the information that was being provided to him by the QMHP in response to how his negative actions affects his family and those in his immediate friend group. Response: The therapy interventions were viewed as ineffective and unproductive, as it was difficult and challenging for client to relax when being instructed by the use of calming techniques provided by QMHP. Client displayed a disinterest in the interventions that were applied to him. Client had to be redirected in regard to the relaxation and calming imageries.
